New client form

Please note that you must schedule an appointment before completing the new client form.

• When you arrive, please check-in with the receptionist.
• Until we bring you into one of our exam rooms, please keep your fur-baby on their leash or in their cage. This is particularly helpful if we need to note their current weight.
• Once we bring you back, one of our veterinary nurses will gather some basic information before our veterinarian will come in to talk with both of you.

1. Payment is due, in full, at the conclusion of the visit or service. WE DO NOT BILL. We accept Care Credit, cash and credit card. We do not extend credit or arrange payment terms. All hospital or surgical estimates are to be paid in full at completion of service.
2. Failure to pay will result in full collection efforts being taken and the customer will be responsible for all collection costs including, but not limited to, court costs, serving by private processor or Sheriff, and any other fees incurred.
